Why We’re Launching This Survey
Empower Simcoe is committed to ensuring that the voices of families are reflected in every stage of support planning. To strengthen that partnership, we are launching a new Individual Support Plan (ISP) survey inviting families to share their experiences, preferences, and ideas for how we can enhance our planning practices.
Families know their loved ones best, and their insights help ensure that ISPs truly reflect each person’s goals, needs, and unique strengths. This survey will help us better understand how families want to participate, where communication can be improved, and how we can make the overall ISP process more collaborative and meaningful across the agency.
Through this feedback, Empower Simcoe aims to refine how information is shared, how meetings are coordinated, and how families engage in planning throughout the year.
What the Survey Covers
The survey focuses on three core areas designed to improve our planning experience and strengthen family involvement:
- Understanding of the Individual Support Plan (ISP) process
- Family preferences for their loved one’s annual ISP review meeting
- How families would like to be involved in ongoing planning and support
Through this feedback, Empower Simcoe aims to refine how information is shared, how meetings are coordinated, and how families engage in planning throughout the year.
Survey timeline
The survey will be open from November 24 to December 8, 2025.
Once it goes live, we will share the link on our website, SharePoint, social media channels, and in the upcoming newsletter.
How Families can participate
If you are a family member of someone receiving supports, we encourage you to take a few minutes to complete the survey below. Every response helps us continue building a more informed, collaborative, and person-centred planning experience.
What Happens Next
Survey results will be compiled, analyzed, and shared in early 2026. The feedback gathered will guide improvements to communication, meeting structure, and long-term planning practices across Empower Simcoe.
