How can we help?

Our Transition Coordinators provide comprehensive support and resources to individuals, their families, schools, and community agencies. We assess skills and abilities, explore interests through the Individual Support Plan, and work towards enhancing decision-making, problem-solving, and daily living skills.

Our goal is to assist individuals in planning, setting, and achieving their personal goals with clear timelines.

Students Supported

It is a coordinated plan developed by schools and community agencies, involving the student and their parent/guardian to facilitate the transition from secondary school to adulthood. These meeting’s start in grade 9 and take place yearly until graduation.

The key goal is for each student to make informed choices and set personal goals that will lead to successful transitions from school into adulthood, whether it be post-secondary education, employment/volunteer opportunities or community living.

Transition Coordinator can help guide students and families/guardians to find resources, services and funding that match their specific needs and interests.

Integrated transition planning helps students with an intellectual/developmental disability and their families to access the services they are entitled to and create a plan for what to do after they leave school.

We aim to provide young adults with the support they need to access post-school options and achieve their goals. Ontario’s Policy/Program Memorandum (PPM) 156 mandates that, for “exceptional students who are age 14 or over and who are not identified solely as gifted, the student’s Individual Education Plan (IEP) must include a transition plan for the student’s transition from school to work, further education, and/or community living.”

TRANSITION PLANNING
TIMELINE

Age 14

  • Transition into Grade 9.
  • Transition Planning Meeting held during school year and will continue to take place every school year.

Age 16

  • Submit application to Development Services Ontario (DSO) for students interested in receiving supports upon completion of High School.
  • Apply for Ontario Photo Card (those without a drivers license).

AGE 17

  • Six months prior to 18th birthday contact Ontario Disability Support Program (ODSP) office in order to apply for Income Support.
  • Application can be made by phone 705-737-1311 or online www.mcss.gov.on.ca.

Age 18-19

  • Apply for GST/HST credit on Income Tax to receive the first payment one month after the 19th birthday.
  • Ensure all appropriate applications and sources of funding have been accessed.

Age 20-21

  • Complete applications to DSO services and supports (Community Participation, Supported Employment, Accommodations and/or Passport Funding).
  • Transition Planning meetings may occur more frequently during the last year of school to ensure a smooth transition.

Christopher has really enjoyed his time with transitions,  the staff have really worked with him to explore things that we as a family had never thought was possible.
